From skilled nursing services to personal care support in the comfort of your own home or at school, our RNs, LPNs and CNAs are highly qualified to offer the best care possible.
Get Help When & Where Its Needed Most
Caring for a medically fragile child or adult is hard, and we understand the challenges your family goes through on a daily basis. Our highly trained nurses and CNAs are here to help around the clock or just when you need it.
Private Duty Nursing
We offer private duty nursing for children and adults. This program is defined by GAPP as a weaning program, where families strive to take over more of the duties involved in their child’s care and return them to a healthier life. We offer care for: ventilator dependencies, tracheostomies, IV therapies, tube feedings, wound care, colostomy care, neurological disorders, respiratory diseases/disorders, transplants, cancer, gastrointestinal disorders, endocrine diseases/disorders, quadriplegic/paraplegic, cardiac defects and other complex needs.
Personal Care Support
Our CNAs (Certified Nursing Assistants) assist medically fragile children and adults with daily living activities such as bathing, dressing, meals and housekeeping around the clock or whenever you need it. Services include: assistance with daily living activities (bathing, meals and housekeeping), monitoring vital signs (including blood pressure, pulse, respiration, temperature), and assistance with ambulation, transfers and more.
When it’s time to go to school and our kids need extra help, ACP is here to help them get stronger and smarter. We partner with the school system to provide one-on-one nursing without interfering with your educational goals. We’re there every step of the day, from getting ready for school and riding the bus to being in the classroom.
Hospital to Home
The transition home with a medically fragile child can be difficult at best, and we can help make this process smoother and more comfortable instead overwhelming. From coordinating the discharge process with the hospital case manager to settling in, from the first time you take your baby home or for a return visit, having the right help in place is essential.
From our management team to our nurses and CNAs, we’re your family’s advocates, helping you with everything from navigating GAPP to coordinating approvals with private insurance.
